To be added to the ASD assessment waitlist, please complete the online submission form below. We expect to contact clients on the waitlist in September/October 2023 with details about appointments/booking dates. We hope to provide ASD assessments starting in October 2023, but this may be subject to change.

Please do not submit duplicate forms. Submitting duplicate forms will not change your position on the waitlist, but may increase administrative processing times.

Please Read the Following Information Regarding ASD Assessments*

The total cost for an ASD assessment is $3000.00. Our agency practices Informed Consent which requires the legal parent(s) or guardian(s) to authorize consent to services. ASD assessments will take place at our Sidney location with a Registered Psychologist. Please note ASD assessments are not available to children under 6 years of age. If the Client has received a psycho-educational assessment from MindKey Health or Dr. Jillian Roberts within the last year, there is a $500 discount for the ASD assessment. Once the appointments are confirmed and booked, there is a non-refundable $250- deposit required to hold your assessment. This deposit is then put towards the overall cost of the assessment.
